After an insertion failure or an expulsion of a Flexi-T, you can ask for a replacement by filling out the replacement form. You can download this form below, form must be completed by a healthcare professional.
Send the completed form with the used IUD (packed in a plastic bag) to your local distributor or to:

If the Flexi-T is no longer in your possession, you (healthcare professional) can also send the replacement form digitally to firstname.lastname@example.org. After receiving the form, we will send you a new Flexi-T free of charge.
Unfortunately, there is no possibility of a refund.
